3.Employment history with addresses & phone numbers for last 24 months                

4. Residence history covering last 24 months

5.Landlord's names, addresses, and phone numbers for last 24 months.             

6.Monthly payment information with account numbers and balances.                                  

7. Lease agreements on any owned rental properties.                                        

8.Bankruptcy papers/Divorce Decrees, if applicable.                                                   

9.Last 2 months Bank statements, investment accounts, 401k etc.                    

10. if Self Employed: Last 2 years tax returns and YTD P/L statements.                                

11. Certificate of Eligibility and DD-214 or Statement of Service (VA only)
